For preschool age children, parents are required to complete a registration form as part of the screening paperwork.  this form will be used for data purposes only. Registration at the base school does not mean that the student is enrolled at that school.

What to bring to the screening:

  - the child for whom there are concerns.

-valid parent/guardian government issued picture id.

-any pertinent medical, developmental or speech/ language reports you have regarding your child.

-the completed registration, referral and/or screening forms that will be sent via email, mail or parent may pick up.

Should further assessment be needed, the parent will be required to bring in the following documentation.

1) Parent will be asked to provide proof of residency in King George, VA.
The following documentation must be provided, and must include
a) one of the following: homeowner/mortgage verification to include mortgage statement, deed, ratified sales agreement; and/or rental agreement/lease;  and
b) one of the following: real estate tax bill/statement, homeowner's/renter's insurance, current utility bill or letter of account from utility company (gas, electric, or water only).
All documents must be current, valid, and reflect the parent's name and address being used.
2) Your child's official, state-issued birth certificate.

Please Note: If proper identification, proof of residency and birth certificate are not available, we will not be able to proceed with the process and/or screening.
